A healthcare provider is seeking an experienced Hospice & Palliative Care Nurse Practitioner to deliver exceptional care in Durham and surrounding areas. The ideal candidate will have a Master’s degree in Nursing, current licensure, and certification as an Adult or Family Nurse Practitioner. Responsibilities include implementing palliative care programs and conducting medical visits. Strong communication and organizational skills are essential for this role, which is part of a team dedicated to patient and family support.

Formación

• Current licensure in the state of practice.
• Experience in Hospice or Palliative Care nursing preferred.
• Knowledge of a hospice care program framework and organization.
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.

Responsabilidades

• Implement palliative care program requirements.
• Complete medical visits per guidelines.
• Conduct face-to-face encounters for re-certification.
• Provide clinical findings of that face to face encounter to PRIMARY CARE PHYSICIAN and supervising Medical Directors.
• Communicate with patients/families at all levels and coordinate with agency personnel.

We are currently seeking an experienced:

HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER

(Covering Durham and surrounding areas)

Responsibilities

• Implements palliative care program requirements
• Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
• Provide clinical findings of that face to face encounter PRIMARY CARE PHYSICIAN, and/or supervising Medical Director/Associate Medical Director
• Communicate with patients/families at all levels
• Communicate with all agency personnel

Qualifications

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program
• Excellent communication and organization skills
