We are currently seeking an experienced:
HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER
(Covering Durham and surrounding areas)
Responsibilities
- Implements palliative care program requirements
- Completes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicated
- Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
- Provide clinical findings of that face to face encounter PRIMARY CARE PHYSICIAN, and/or supervising Medical Director/Associate Medical Director
- Communicate with patients/families at all levels
- Communicate with all agency personnel
Qualifications
- Master’s degree in Nursing required, with current licensure in the state of practice
- Certification as an Adult or Family Nurse Practitioner
- Experience in Hospice or Palliative Care nursing preferred
- Knowledge of the framework, function, and organization of a hospice care program
- Excellent communication and organization skills
