Job summary

Liberty Hospice in Wilmington, NC is seeking a Palliative Care RN for a part-time role. You will review referrals, provide expert, individualized coordination of care for patients and families, and complete comprehensive physical and psychosocial assessments to identify needs and intervention areas.

You will support adherence to treatment plans, arrange referrals to specialists when indicated, assist patients in accessing community resources, and document assessments, interventions, responses,

Qualifications

  • Requires current RN licensure in state of branch location and three years of clinical nursing experience, preferably in Home Care, Hospice or Skilled Nursing.
  • Ability to independently organize work, communicate patient needs, provide patient education and coordinate visit schedules.
  • Willingness to travel and work under assigned conditions.
  • Must have CPR certification and strong written/oral communication skills.
  • Minimum of twelve hours of continuing education in a twelve-month period; valid driver's license.

  • Assist Nurse Practitioners with day-to-day program tasks and tracking palliative referrals.
  • Alleviate pain, fatigue, nausea, and other distressing symptoms.
  • Accurately document patient assessments, interventions, responses, needs, goals, and teaching in the EHR.

JOB REQUIREMENTS:
  • Requires current RN licensure within state of branch location and three years of clinical nursing experience, preferably in a Home Care, Hospice or Skilled Nursing facility setting. Successfully completed nursing education from an approved school of nursing.
  • Must be able to independently organize work, communicate patient needs, and provide patient education and coordinate patient visit schedule(s).
  • Must accept travel and working conditions as assigned.
  • Must possess effective communication skills (oral and written), as well as, CPR certification.
  • Minimum of twelve hours of continuing education in a twelve-month period. Must have a valid driver's license.
