Job summary

Liberty Hospice in Greenville, NC is seeking an experienced Hospice & Palliative Care Nurse Practitioner for a part-time role (2-3 days/week). You will implement palliative care programs, conduct patient visits, and provide recertification information to medical leadership.

Required: Master’s in Nursing, current NC NP licensure, and certification as Adult/Family NP. Hospice experience and strong communication are preferred; background checks and a drug-free workplace are standard.

Qualifications

• Master’s degree in Nursing required, with current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner.
• Experience in Hospice or Palliative Care nursing preferred.
• Knowledge of the hospice care program structure and operations.
• Excellent communication and organizational skills.

Responsibilities

• Implements palliative care program requirements.

• Conduct face-to-face encounters to provide recertification information to Medical Director/Associate Medical Director for Hospice Patients.

• Conduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice Patients
• Provide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical Director
• Communicate with patients/families at all levels.
• Communicate with all agency personnel

Job Requirements

• Master’s degree in Nursing required, with current licensure in the state of practice
• Certification as an Adult or Family Nurse Practitioner
• Experience in Hospice or Palliative Care nursing preferred
• Knowledge of the framework, function, and organization of a hospice care program.
• Excellent communication and organization skills.
