Job summary

CleanSlate Centers in Kokomo, IN seeks a Nurse Practitioner to deliver medication-assisted treatment for opioid and alcohol use disorders. You will work with a multidisciplinary team to provide patient-centered care and ensure timely documentation.

Responsibilities include evaluating patients, creating individualized plans, and collaborating with physicians. Telemedicine support and regional coverage may be required.

Qualifications

• Active NP license and DEA registration in state of employment.
• Graduate of an accredited Nurse Practitioner program.
• Strong interpersonal and critical thinking skills.

Responsibilities

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi.
• Evaluate patients and determine individualized treatment plans.
• Monitor patient progress and adjust medications as needed.
• Collaborate with physicians and care teams on complex cases.
• Maintain accurate documentation in the EMR within 48 hours.
• Participate in clinical meetings, training, and quality improvement efforts.
• Support telemedicine and regional coverage as needed.
