Job summary

CleanSlate Centers is seeking a Nurse Practitioner to lead medication-assisted treatment for patients with opioid and alcohol use disorders. You will administer buprenorphine, Vivitrol, Sublocade, and Brixadi alongside a multidisciplinary team to deliver patient-centered care.

The role emphasizes evaluating and adjusting treatment plans, documenting promptly in the EMR, and participating in quality improvement initiatives.

Qualifications

• Active NP license and DEA registration in state of employment.
• Graduate of an accredited Nurse Practitioner program.
• Experience in addiction medicine or behavioral health preferred.
• Comfort with EMRs and a fast-paced clinical setting.

Responsibilities

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi.
• Evaluate patients and determine individualized treatment plans.
• Monitor patient progress and adjust medications as needed.
• Collaborate with physicians and care teams on complex cases.
• Maintain accurate documentation in the EMR within 48 hours.
• Participate in clinical meetings, training, and quality improvement efforts.
• Support telemedicine and occasional regional coverage as needed.
