Job summary

CleanSlate Centers in Greencastle, IN is seeking an Experienced Nurse Practitioner to join our MAT program and deliver high-quality, patient-centered care.

You will provide medication-assisted treatment for opioid and alcohol use disorders, collaborate with a multidisciplinary team, monitor progress, and document in the EMR within 48 hours. This full-time, day-shift role offers strong benefits and growth opportunities in a mission-driven setting.

Qualifications

• Active NP license and DEA registration in the state of employment.
• Graduate of an accredited Nurse Practitioner program.
• Strong interpersonal and critical thinking skills.
• Experience in addiction medicine or behavioral health preferred.
• Comfort with EMRs and a fast-paced clinical setting.

Responsibilities

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi
• Evaluate patients and determine individualized treatment plans
• Monitor patient progress and adjust medications as needed
• Collaborate with physicians and care teams on complex cases
• Maintain accurate documentation in the EMR within 48 hours
• Participate in clinical meetings, training, and quality improvement efforts
• Support telemedicine and occasional regional coverage as needed

What You'll Do:
