Job summary

CleanSlate Centers is seeking an experienced Nurse Practitioner at our Alexandria, IN clinic to lead addiction medicine care with medication-assisted treatment and collaborative physician support.

The role emphasizes patient-centered MAT, documentation in EMR, and participating in clinical quality initiatives. Monday–Friday schedule with comprehensive benefits and CME support is offered.

Qualifications

• Active NP license and DEA registration in state of employment.
• Graduate of an accredited Nurse Practitioner program.
• Experience in addiction medicine or behavioral health preferred.

Responsibilities

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi.
• Evaluate patients and determine individualized treatment plans.
• Monitor patient progress and adjust medications as needed.
• Collaborate with physicians and care teams on complex cases.
• Maintain accurate documentation in the EMR within 48 hours.

What You’ll Do:

As a Nurse Practitioner at CleanSlate, you’ll play a vital clinical role in treating patients with opioid and alcohol use disorders through medication-assisted treatment (MAT). You’ll work alongside a team of dedicated medical and behavioral health professionals to ensure high-quality, patient-centered care.

Key Responsibilities:

• Provide medication-assisted treatment using buprenorphine, Vivitrol, Sublocade, and Brixadi
• Evaluate patients and determine individualized treatment plans
• Monitor patient progress and adjust medications as needed
• Collaborate with physicians and care teams on complex cases
• Maintain accurate documentation in the EMR within 48 hours
• Participate in clinical meetings, training, and quality improvement efforts
• Support telemedicine and occasional regional coverage as needed

• Active NP license and DEA registration in state of employment
• Graduate of an accredited Nurse Practitioner program
• Strong interpersonal and critical thinking skills
• Experience in addiction medicine or behavioral health preferred
• Comfort with EMRs and a fast-paced clinical setting
