Job summary

St. Croix Hospice is seeking a compassionate Nurse Practitioner to support patients and families at the end of life. You will provide medical care, manage medications, and guide decision‑making with dignity and respect.

Responsibilities include collaborating with physicians and care teams, completing hospice recertifications, and educating families on medical and emotional needs while maintaining HIPAA compliance.

Qualifications

• Hold an NP or APRN license in the practicing state(s).
• Degree from an accredited NP program is required.

Qualifications

Nurse Practitioner Requirements & Key Responsibilities

• Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face‑to‑face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end‑of‑life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem‑solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
