Job summary

St. Croix Hospice seeks a compassionate Nurse Practitioner to support patients and families facing advanced illness. You will provide skilled medical care, oversee medications and recertifications, and guide care plans in coordination with a multidisciplinary team.

The role emphasizes autonomy, patient-centered decision making, and clear communication with families, with HIPAA-compliant documentation and opportunities for professional growth in hospice care.

Qualifications

• Hold a current NP or APRN license in state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; hospice or palliative care preferred.
• Provide comprehensive medical care for terminally ill patients, including assessments and care plans.
• Prescribe medications and complete hospice recertifications, compliant with regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms.
• Educate/support patients, families, and caregivers on end‑of‑life care.
• Maintain documentation, adhere to HIPAA, participate in quality initiatives.
• Demonstrate clinical judgment, communication, and problem‑solving; proficient with EHR.
• Valid driver’s license and transportation; pass background/drug screening.

• Prescribe medications/treatments, complete hospice recertification face‑to‑face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
