Job summary

St. Croix Hospice is seeking a compassionate Nurse Practitioner to join our team in Wisconsin. You will provide skilled medical care and guidance to patients and families navigating advanced illness, with emphasis on comfort, dignity, and autonomy.

In this role, you will oversee medications, treatments, and hospice recertifications, collaborate with interdisciplinary teams, and educate families on end-of-life care while maintaining HIPAA-compliant documentation.

Qualifications

• Must have current NP license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; hospice/palliative care preferred.
• Provide comprehensive medical care for terminally ill patients.
• Prescribe medications and treatments; complete hospice recertifications and ensure regulatory compliance.
• Collaborate with interdisciplinary teams and physicians to manage symptoms.
• Educate patients, families, and caregivers on end-of-life care.
• Maintain accurate documentation; HIPAA compliance; proficient in EHR.
• Valid driver’s license and reliable transportation; pass background and drug screening.

Nurse Practitioner Requirements & Key Responsibilities

• Must have current Nurse Practitioner (NP) license in the state(s) of practice.

• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.
