Job summary

St. Croix Hospice in Moline, IL is seeking a compassionate Nurse Practitioner to support patients and families during the final stages of illness with thoughtful medical care and a steady presence.

The NP will manage medications, treatments, and hospice recertifications, collaborating with interdisciplinary teams to ensure comfort, dignity, and patient goals are met. Strong communication and HIPAA-compliant documentation are essential.

Qualifications

• Hold a current NP/APRn license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments and care plans.
• Prescribe medications and complete hospice recertifications in compliance with regulations.

QualificationsNurse Practitioner Requirements & Key Responsibilities

• Hold a current Nurse Practitioner (NP) or Advanced Practice Registered Nurse (APRN) license in the state(s) of practice.
• Degree from an accredited NP program required.
• Minimum two years of RN experience; preferred hospice or palliative care experience.
• Provide comprehensive medical care for terminally ill patients, including assessments, diagnostic interpretation, and individualized care plans.
• Prescribe medications/treatments, complete hospice recertification face-to-face assessments, and ensure compliance with state and federal regulations.
• Collaborate with interdisciplinary teams and physicians to manage symptoms and maintain continuity of care.
• Educate and support patients, families, and caregivers on end-of-life care, including medical, emotional, and spiritual needs.
• Maintain accurate documentation, adhere to HIPAA standards, and participate in quality improvement initiatives.
• Demonstrate strong clinical judgment, communication, and problem-solving skills; proficient in electronic health records.
• Valid driver’s license, reliable transportation, automobile insurance, and ability to pass background and drug screening required.

The salary for this role is: $35 per hour for orientation, meetings and required education and $100 pe face to face visit. An employee’s actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
