Job summary

St. Croix Hospice seeks a Nurse Practitioner to provide comprehensive hospice care under physician supervision.

You will assess patients’ health history, develop and direct plans of care, prescribe medications, and coordinate with the hospice team to deliver high-quality end-of-life care. You will conduct thorough assessments, ensure compliance with regulations, and work within an interdisciplinary team to maximize comfort and quality of life for patients and families in a home or facility

Qualifications

• Must have current NP license in state of practice.
• Degree from accredited nursing/Nurse Practitioner program.
• Minimum two years RN experience.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License and reliable vehicle.

Responsibilities

• Provide medical care and symptom management for hospice patients under physician supervision.
• Develop and adjust patient plans of care, prescribe medications or treatments as needed.
• Collaborate with interdisciplinary team to coordinate care and ensure accurate documentation.
• Conduct face-to-face assessments for eligibility and recertification as required.
