What you’ll do
As an NP/PA SNF-ist, you will serve as a primary internal medicine provider for residents in the skilled nursing setting.
Responsibilities include:
- Manage the medical care of short-term rehabilitation and long-term care residents
- Perform comprehensive health assessments and physical examinations
- Diagnose and treat acute and chronic medical conditions
- Prescribe medications and treatments within your scope of practice
- Evaluate residents experiencing changes in condition
- Conduct routine and monthly stability visits
- Participate in advanced care planning and goals-of-care discussions
- Collaborate with nursing staff, medical directors, attending physicians, specialists, and interdisciplinary care teams
- Facilitate referrals to specialists and other healthcare providers as appropriate
- Maintain accurate clinical documentation and CPT coding
- Follow established clinical protocols and best practices
- Help identify opportunities to prevent avoidable emergency department visits and hospitalizations
What you’ll need
Required:
- Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina
- Master's degree from an accredited Nurse Practitioner or Physician Assistant program
- Strong clinical judgment and communication skills
- Ability to work independently while collaborating effectively with facility-based care teams
- Strong organizational and documentation skills
Preferred:
- At least 1 year of experience in a Skilled Nursing Facility (SNF), long-term care, post-acute care, or similar setting
- Experience caring for geriatric or medically complex populations
- Experience managing acute and chronic conditions in a post-acute setting
Available to full-time employees working at least 30 hours per week. Benefits become effective following a 30-day eligibility period, on the first of the month thereafter.
