Job summary

This full-time role emphasizes meaningful, longitudinal patient relationships and strong collaboration with the care team. The position combines in-person care with virtual charting in a hybrid model, offers a sign-on/retention bonus up to $15,000, and a comprehensive benefits package including

Qualifications

• Active, unencumbered NP or PA license in North Carolina.
• Master's degree from an accredited NP/PA program.
• Strong clinical judgment and communication skills.
• Ability to work independently while collaborating with facility teams.
• Strong organizational and documentation skills.

Responsibilities

• Provide primary internal medicine care for SNF residents.
• Perform comprehensive health assessments and exams.
• Diagnose and treat acute and chronic conditions.
• Prescribe medications within scope.
• Manage changes in patient condition and conduct stability visits.
• Participate in advanced care planning discussions.
• Collaborate with nursing staff, medical directors, and specialists.
• Facilitate referrals to other providers as needed.
• Maintain accurate documentation and CPT coding.
