Job summary

Telos Health Systems is seeking an experienced Advanced Practice Provider—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to serve as an in-house SNF-ist, delivering general medicine care to geriatric residents in Skilled Nursing Facilities and Assisted Living Facilities in the Brevard, NC area.

This role offers autonomy, collaborative care, and the opportunity to make a meaningful impact in long-term care for underserved populations.

Qualifications

• Active, unencumbered NP or PA licensure in North Carolina.
• Current DEA registration.
• Master’s degree from an accredited NP or PA program.
• Strong communication and clinical leadership skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Serve as the facility’s primary internal medicine provider (SNF-ist).
• Manage care for short-term rehab and long-term residents.
• Perform comprehensive health assessments, physical exams, and diagnoses.
• Prescribe medications and treatments as appropriate.
• Collaborate with interdisciplinary teams and attending physicians.
• Complete accurate documentation and CPT coding.
• Facilitate referrals to specialists as needed.

Compensation: *Offering up to $7,500 sign-on bonus* Assumi ng Full-Time 75 visits/week (15 visits/day), a primary care APP could estimate they will earn $120k - $140k/year.

Telos Health Systems is seeking an experienced Advanced Practice Provider — either a Nurse Practitioner (NP) or Physician Assistant (PA) — to serve as an in-house SNF-ist, providing general medicine care to geriatric residents in Skilled Nursing Facilities (SNFs) and Assisted Living Facilities (ALFs) in the Brevard, NC area.

What You’ll Do

• Serve as the facility’s primary internal medicine provider (SNF-ist)
• Manage care for short-term rehab and long-term care residents
• Perform comprehensive health assessments, physical exams, and diagnoses
• Manage acute and chronic conditions following established protocols
• Prescribe medications and treatments as appropriate
• Conduct monthly stability visits for long-term residents
• Evaluate residents with changes in condition
• Participate in Advanced Care Planning
• Collaborate with interdisciplinary teams, medical directors, and attending physicians
• Complete accurate documentation and CPT coding
• Facilitate referrals to specialists and other healthcare providers

What You’ll Need

Required

• Active, unencumbered Nurse Practitioner (APRN/APN) or Physician Assistant (PA-C) license in North Carolina
• Current DEA registration
• Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
• Strong communication and clinical leadership skills
• Ability to work effectively in a fast-paced environment

Preferred

• At least 1 year of experience in a Skilled Nursing Facility (SNF) or long-term care setting
• Experience caring for geriatric populations
