Job summary

Wellstar Health System is seeking a Nurse Navigator (RN) to guide patients and families through cancer care with disease-specific expertise and compassionate, coordinated support.

This role interfaces with patients in person, by phone, and electronically, ensuring timely access to services, collaborative care planning, and adherence to care plans within Wellstar's integrated specialty programs.

Qualifications

• Associates degree in nursing or diploma; BSN preferred.
• Current RN license in Georgia; BLS certification.
• Minimum 3 years nursing practice; 2 years disease-specific experience preferred.
• Outpatient care or care coordination experience preferred.
• Ability to function in fast-paced environment; strong critical thinking.

Responsibilities

• Serve as subject matter expert in nurse navigation and improve care timeliness and access.
• Coordinate care across specialty clinics and interdisciplinary rounds.
• Collaborate with navigation team to support program growth and performance improvement.
• Provide patient-centered care tailored to diverse populations.
• Maintain documentation and effective communication with care teams.
