Job summary

Wellstar Health Systems is seeking a Nurse Navigator to guide patients, families, and caregivers through the cancer care continuum. The role focuses on timely access to care, education, and resources, interfacing in person, by phone, and electronically, outside bed/chair side duties.

The NN will coordinate care, document activities, support program growth, and collaborate with navigation teams to identify gaps and bridge resources for patients across the cancer continuum.

Qualifications

• Minimum 5 years nursing practice.
• Minimum 2 years nursing with disease-specific experience.
• Outpatient care or care coordination experience preferred.

Responsibilities

• Serve as subject matter expert in patient navigation and improve care timeliness and access.
• Coordinate care and documentation across interdisciplinary teams and clinical settings.
• Precept and mentor other nurses to ensure workforce readiness.
• Promote evidence-based nursing practice and performance improvement.
• Coordinate care across specialty clinics and multidisciplinary rounds.
