Job summary

Qualifications

• High School Diploma or GED required.
• 2+ years in healthcare setting, patient scheduling or care coordination.
• Strong computer literacy and excellent organizational and communication skills.

Responsibilities

• Coordinate patient appointments across multidisciplinary care teams and refer to internal/external resources as needed.
• Act as liaison between patient, nurses, physicians, and other care team members to ensure smooth care transitions.
• Develop a comprehensive care plan based on individual patient needs and preferences.
