Qualifications

Responsibilities

• Provides language-specific education, psychosocial support, and necessary resources for newly diagnosed patients and families, coordinating referrals as needed.
• Educates patients and families about diagnosis, work-up, treatments (including surgery), survivorship issues, and follow-up.
• Serves as liaison for patients and families between departments to ensure smooth care.
• Coordinates new patient referrals, scheduling of exams and appointments, and identifies gaps in care for patients returning home for treatment.
• Contacts every newly diagnosed patient at the time of diagnosis.
• Assesses emotional and social needs and barriers to care (insurance, transportation) and refers to resources.
• Identifies barriers to timely care and supports service recovery and patient satisfaction.
