Job summary

Advocate Health in Charlotte, NC is seeking a registered nurse to support thoracic surgery patients through diagnosis, work-up, treatments and follow-up. You will educate patients and families, coordinate referrals, schedule exams and advocate for timely care across departments.

BSN is required with MSN preferred; current NC RN licensure is necessary and certification in the specialty within 2 years is expected.

Qualifications

• BSN degree required; MSN preferred.
• Current licensure to practice as a Registered Nurse in the state.
• Certification in the appropriate specialized field within 2 years of being in the position required.
• Excellent communication skills required.

Responsibilities

• Provides language specific education, psychosocial support, and resources for newly diagnosed patients and coordinates referrals.
• Educates patients and families regarding diagnosis, work-up, treatments, including surgery and follow-up.
• Serves as liaison for patients and families between departments.
• Coordinates new patient referrals, scheduling of exams, procedures and appointments, and follow-up care.
• Contacts every newly diagnosed patient at the time of diagnosis.
• Assesses emotional and social needs and barriers to care, referring to needed resources.
• Identifies barriers to expedient care and conducts service recovery to ensure patient satisfaction.
