Job summary

Mercy is seeking an RN Care Manager to facilitate discharge planning from admission through discharge. You will collaborate with the care team to coordinate patient throughput and ensure safe, effective transitions.

The role requires a BSN, current Missouri RN license, and 2–3 years in acute care. Care management or utilization management experience is preferred; Case Management certification is a plus.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in Missouri required.
• Bachelor of Science in Nursing (BSN) is required.

Responsibilities

• Facilitate discharge planning from admission to discharge in collaboration with the healthcare team.
• Coordinate patient throughput and transitions with the care management team.
• Support care management needs for patients with complex or high-risk psychosocial issues.
