Job summary

Hackensack Meridian Health is seeking a Palliative Nurse Navigator to collaborate with palliative care providers to deliver quality and specialty palliative care to patients and their loved ones.

The role includes ongoing assessment of patient and family needs across the 8 domains of palliative care and connecting patients and families to available resources within the health system and in the community. RN with at least 5 years of clinical experience required.

Qualifications

• RN with at least 5 years of clinical experience in a relevant specialty.
• Graduate of a licensed school of nursing.
• Proficient with Google, Microsoft, and EPIC.
• Excellent written and verbal communication skills.
• Strong organizational and time-management abilities.

Responsibilities

• Collaborate with palliative care providers to deliver quality care.

The Palliative Nurse Navigator in collaboration with the palliative care providers/clinicians providing quality and specialty palliative care to patients and their loved ones. This care includes: ongoing assessment of patient and family needs including the 8 domains of palliative care and connecting patients and families to available resources within the health system and in the community.

Education, Knowledge, Skills and Abilities Required:

• Graduate of licensed school of nursing;
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills- proficiency in Google, Microsoft, EPIC
• Strong organizational, time management, critical thinking RN skills.
• Ability to work independently and exercise sound judgment in interactions with providers,
• Clinicians, patients, and their families.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.

Education, Knowledge, Skills and Abilities Preferred:

• BSN

Licenses and Certifications Required:

• NJ State Professional Registered Nurse License.
• AHA Basic Health Care Life Support HCP Certification.

Licenses and Certifications Preferred:

• Certified Hospice and Palliative Nurse.
