Munson Healthcare in Petoskey, MI is seeking a Nurse Navigator at the Petoskey Specialty Clinic. This full-time, day-shift role coordinates care for adults and pediatrics with complex conditions, serving as the liaison between patients, clinical staff and physicians.

You will participate in care management, support quality improvement, and work with primary care providers to deliver cost-effective health care while promoting patient education and a positive patient experience.

Qualifications

• RN license in Michigan required.
• Experience with complex chronic disease management preferred.
• Bachelor of Science in Nursing preferred.
• Five years’ RN experience preferred.
• Vaccination requirements for Munson Healthcare staff.

Responsibilities

• Act as a liaison between patients, staff and physicians to provide care management and coordination for adults and pediatrics with complex conditions.
• May work on the clinical side if needed.
• Collaborate with primary care providers, specialists, and families to deliver quality and cost-effective health care.
• Assess plans, implement, coordinate, monitor and evaluate all options to optimize health status and reduce hospital stays.
• Support quality improvement initiatives and adherence to safety standards.
