Job summary

Munson Healthcare seeks an experienced Registered Nurse to serve as a liaison among patients, families and the care team, coordinating care for adults and children with complex conditions.

You will assess, implement, and monitor plans to optimize health, reduce hospital use, and support safety and quality initiatives within a hospital setting.

Qualifications

• Licensed RN in the State of Michigan.
• Experience in complex chronic disease management preferred.
• Bachelor of Science degree in nursing preferred.
• Five years' (5) experience as an RN preferred.
