Job summary

Munson Healthcare Careers in Michigan is hiring an RN to join our team. The role focuses on care management and coordination for adult and pediatric patients, working between Indian River and Charlevoix locations with a Monday–Friday, 8 AM–4 PM schedule.

Qualifications

• Licensed RN in the State of Michigan.
• Experience in complex chronic disease management preferred.
• Bachelor of Science degree in nursing preferred.
• Five years of RN experience preferred.

Responsibilities

• Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.
• Collaborate with primary care providers, specialists, and patient/families to ensure delivery of quality and cost-effective health care services.
• Assess plans, implement, coordinate, monitor and evaluate all options and services to optimize the patient's health status and decrease hospital and emergency room utilization.
• Support quality improvement initiatives and adherence to safety standards.
