Responsibilities

• Act as a liaison between patients, professional staff and physicians to provide care management and coordination for adult and pediatric patients with complex conditions and needs.
• Collaborate with primary care providers, specialists, and patient/families to ensure delivery of quality, cost-effective health care services.
• Assess plans, implement, coordinate, monitor and evaluate options to optimize the patient's health status and reduce hospital and ER utilization.
• Support quality improvement initiatives and adherence to safety standards.

• Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services.

• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.

Qualifications

What’s Required

• Licensed RN in the State of Michigan

• Experience in complex chronic disease management preferred

• Bachelor of Science degree in nursing preferred

• Five years' (5) experience as an RN preferred
