Job summary

Munson Healthcare in Michigan is seeking a licensed RN to coordinate care for adults and pediatrics with complex conditions. The role focuses on care management, collaboration with physicians and families, and improving health outcomes.

Qualified candidates will hold a Michigan RN license, prefer a BSN, and have 5+ years of RN experience with chronic disease management. Travel to practices may be required as needed.

Qualifications

• Five years' RN experience preferred.

Responsibilities

• Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions.
• Collaborate with primary care providers, specialists, and patient/families to ensure delivery of quality, cost-effective health care services.
• Assess plans, implement, coordinate, monitor and evaluate options and services to optimize patient health status and reduce hospital and ER utilization.
• Support quality improvement initiatives and adherence to safety standards.

Job Description

• Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.
• Support quality improvement initiatives and adherence to safety standards.

Qualifications

• Licensed RN in the State of Michigan
• Experience in complex chronic disease management preferred
• Bachelor of Science degree in nursing preferred
• Five years' (5) experience as an RN preferred
