Munson Healthcare in Traverse City, MI is seeking a Nurse Navigator to coordinate care for adults with complex needs. The role involves collaborating with clinicians, coordinating services, and promoting efficient, patient-centered care within a large regional health system.

The navigator acts as a liaison among patients, families and the care team, ensuring comprehensive planning, education, and continuity of care across settings.

Qualifications

• RN licensed in Michigan.
• BSN preferred.
• Experience in Pre-Admission Testing, Case Management or relevant clinical experience.
• Experience in complex chronic disease management preferred.
• Five years' RN experience preferred.

Responsibilities

• Act as liaison between patients, staff, and physicians to provide care management and coordination.
• Collaborate with primary care providers, specialists, and families to ensure quality, efficient care.
• Assess, plan, implement, coordinate, monitor and evaluate options to optimize health and reduce hospital/ER use.
• Develop patient-centered care plans using evidence-based guidelines.

A Day inthe Life

The navigator functions in an organized and time conscious manner using effective customer service and interpersonal skills at all times.

• The Nurse Navigator will act as a liaison between patients, professional staff and physicians by providing care management and care coordination for adultpatients, with complex conditions, with complex social needs, and education needs.
• Serves in an expanded health care role to collaborate with primary care providers, specialists, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status and decreasing hospital and emergency room utilization.
• Integrates evidence-based clinical guidelines and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care

Qualifications

What’s Required

• Currently licensed as a Registered Nurse in the State of Michigan.
• Bachelor of Science degree in nursing preferred.
• Previousexperience in Pre-Admission Testing, CaseManagementor clinical experience relevant to patient population being served is necessary.
• Experience in complex chronic disease management preferred
• Bachelor of Science degree in nursing preferred
• Five years' (5) experience as an RN preferred

Will split time between Annual Wellness Visits and Care Management
