Job summary

American Medical Administrators LTC is seeking a Nurse Assessment Coordinator in Hermann, MO to oversee the assessment and care planning process for residents in accordance with federal and state guidelines.

You will coordinate MDS assessments, develop individualized care plans, and ensure accurate documentation for reimbursement and high-quality care. Collaboration with interdisciplinary teams and education of staff on MDS guidelines are key parts of the role.

Responsibilities

• Coordinate and complete accurate and timely MDS assessments and care plans for all residents.
• Ensure compliance with federal, state, and company regulations.
• Collaborate with interdisciplinary teams to develop individualized care plans.
• Monitor data for trends, accuracy, and quality assurance.
• Participate in care plan meetings, audits, and surveys as needed.
• Educate staff on documentation best practices and changes to MDS guidelines.
