Job summary

Heritage Woods of Canton is seeking an experienced Nursing Supervisor to lead the clinical team in delivering compassionate resident care. You will conduct assessments, develop care plans, oversee medication management, and coordinate infection control while ensuring family and physician communication.

This full-time role requires an active Illinois nursing license (LPN required, RN preferred) with 2+ years of supervisory experience and a proven ability to mentor staff in a supportive,

Responsibilities:Conducts initial assessment of prospective residents, including off-site visitsPerforms wellness assessments and develops individualized care plans based on the needs and preferences of each residentDelivers health promotion services, offers disease prevention counseling, and provides self-care education to residentsApproximately 20% of hours will be providing hands-on nursing responsibility and direct careEngages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experienceFosters communication by regularly engaging with physicians, pertinent healthcare providers, and family membersHires, trains, schedules, and leads the clinical team in providing exceptional care experiencesCoordinates the Community’s infection control program while maintaining state regulatory complianceDevelop and oversee the Community’s medication management programContributes toward quality assurance and performance improvement initiativesComplies with all rules and regulations regarding confidentiality and privacy of resident informationQualifications:Maintains current state nursing licensure.
