Job summary

Brookstone Estates is seeking a nursing supervisor to conduct resident assessments, create individualized care plans, and coordinate wellness services while leading a clinical team in a compliant, compassionate environment.

You will collaborate with physicians and families, oversee infection control and medication management, and drive quality improvement initiatives. LPN licensure is required, RN preferred, with at least two years of supervisory nursing experience.

Qualifications

• Maintains current state nursing license; LPN required, RN preferred.
• Minimum of two years nursing supervisory experience.
• Senior/supportive living experience strongly preferred.
• Excellent written and verbal communication skills; leadership ability.
• Ability to mentor, motivate, and inspire team members.
• Demonstrates exceptional interpersonal skills with compassion and dignity.

Responsibilities

• Conducts initial assessment of prospective residents, including off-site visits.
• Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident.
• Delivers health promotion services, disease prevention counseling, and self-care education to residents.
• Provides hands-on nursing responsibility and direct care as needed (approximately 20% of hours).
• Engages with residents and family members in a compassionate and impactful manner.
• Fosters communication with physicians, healthcare providers, and family members.
• Hires, trains, schedules, and leads the clinical team to deliver exceptional care.
• Coordinates the infection control program to maintain state regulatory compliance.
• Develops and oversees the community’s medication management program.
• Contributes to quality assurance and performance improvement initiatives.
• Complies with confidentiality and privacy rules and regulations.
