Job summary
Washington State Department of Social and Health Services is seeking a Registered Nurse 4 to oversee the Medical Discharge Program. The RN4 will supervise 3 nursing staff, coordinate facility visits, and guide discharge planning per best practices. The role includes medical consultation and quality assurance oversight.
Position is based on McNeil Island with ferry travel from Steilacoom provided. On-site work at least three days weekly may earn an extra 5% premium pay; employees receive a daily
Qualifications
- Graduation from an accredited school of professional nursing.
- Four years of professional nursing experience including two years in a lead capacity.
- Bachelor's degree or higher in nursing may substitute for two years of non-supervisory experience.
- One year of full-time teaching nursing may substitute for one year of supervisory experience.
Responsibilities
- Coordinate and delegate site visits to facilities for resident placement and review assessments.
- Audit facilities to ensure resident needs are met and report to Medical Director and leadership.
- Track ADL and Medication assessments and train residents on medication identification and use.
- Supervise nursing activities for conditionally released residents on a 24-hour basis.
- Develop wellness plans and advise on weight loss, exercise, nutrition, and chronic condition management.
- Audit Keep on Person medications for residents nearing discharge.
