Job summary

Harborview Medical Center, part of UW Medicine, seeks a per diem Registered Nurse to join the Care Management Team in Seattle, WA. The role features variable day shifts and potential weekend work within a per diem/intermittent schedule.

Requirements include two years inpatient nursing experience, one year in case management or discharge planning, WA RN license, and BLS certification. This position supports patient transitions and collaboration across care teams.

Qualifications

• Two years of recent inpatient nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent experience
• One year of Case Management or discharge planning experience and understanding of care coordination and transitions of care
• Current license to practice as a registered nurse in the State of Washington
• Current BLS certification

Responsibilities

• Care Management team coordinates all aspects of in-patient care leading to timely transitions to the next level of care
• Serve as liaison between patients, families, medical teams, nursing staff, social services and agencies involved from admission to discharge
