Job summary

Responsibilities

• Manage assignment of complex cases on adult floors.
• Collaborate with Social Worker, Physician advisor, and Care Coordination leadership.
• Provide leadership and advocacy in patient-centered care across the care continuum.
• Oversee transition planning and identify/eliminate barriers to care delivery.
