Job summary

AccentCare is seeking a Home Health Registered Nurse Case Manager in Woodward, OK. The role oversees patient care plans from admission to discharge, coordinating direct and indirect care, and guiding nurses, LPNs, and CHHAs to implement individualized plans of care.

The position demands RN licensure in the operating state, at least 1 year of RN experience, and CPR certification, with a pay range reflecting years of experience. On-site schedule typically Monday–Friday; monthly on-call.”

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice in the state of operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsibilities

• Assess the status of nursing needs of assigned patients and overall management of the patient’s care plan.
• Complete initial patient assessment upon admission, including patient history.
• Selects appropriate nursing diagnosis and establish measurable goals.
• Inform physician of assessment results and recommended interventions.
• Implement nursing interventions and adjust care as needed within scope of practice.
• Re-evaluate patients’ nursing and clinical needs as required.
• Document assessments, progress, and discharge notes in a timely fashion.
