Job summary

Luke Air Force Base is seeking a Registered Nurse - Case Management to oversee care coordination and discharge planning for patients across inpatient and outpatient settings. You will assess needs, develop care plans, and coordinate with the healthcare team and community resources to ensure timely, quality care for service members and families.

Responsibilities include implementing care plans, educating patients and caregivers, and monitoring outcomes in alignment with DoD and service

Qualifications

• Requires Associate or Bachelors degree in Nursing from ACEN/CCNE accredited program.
• Current RN license in good standing; life support (BLS) certification required.
• Certification as Case Manager (CCM/ANCC) preferred or eligible with 3 years' RN case management experience.

Responsibilities

• Provide case management, care coordination, and discharge planning for inpatient and outpatient care.
• Assess patient and family needs; develop and implement individualized care plans.
• Coordinate with healthcare team and community resources to ensure timely services.
• Educate patients and families; monitor treatment plans and outcomes; document findings.
