Job summary

Qualifications

• RN license in good standing with a current, unrestricted status
• ACEN/CCNE-accredited nursing education
• Three years of RN case manager experience within the last four years
• Citizen of the United States

Responsibilities

• Participate in all phases of the Case Management Program (CMP) and ensure care standards
• Provide nursing expertise in assessment, planning, implementation, coordination, and monitoring
• Develop and implement CM tools and processes for patient identification and care plans
• Coordinate CM with utilization management and social work when appropriate
• Maintain liaison with community agencies and document patient data accurately
