Job summary

Luke & Associates, Inc. is seeking a Registered Nurse Case Manager to support Intrepid Spirit Centers under a Defense Health Agency contract.

The role focuses on patient management, care coordination, and discharge planning for outpatient settings addressing TBI, combat stress, and related psychological health needs. Site of service includes Fort Belvoir, Camp Lejeune, Camp Pendleton, Fort Campbell, JBLM and NICoE in Bethesda; shifts are Monday to Friday with 40 hours per week.

Qualifications

• Graduate from ACEN/CCNE-accredited program; BSN preferred.
• Must hold active RN license in any US state.
• At least one year nursing experience post-graduation.

Responsibilities

• Provide patient management, care coordination, and discharge planning for outpatient settings.
• Coordinate care with multiple providers across sites and disciplines.
• Document outcomes and maintain notes in MHS Genesis within 3 business days.
• Respond to beneficiary messages in MHS Genesis within 1 business day.
• Demonstrate leadership in managing complex patient care.
