Job summary

Clinica Family Health & Wellness (CFHW) is hiring an RN Case Manager for MAT and SUD services in Colorado. You will work with an interdisciplinary team to support patients through MAT, withdrawal management, and outpatient services.

The role emphasizes recovery-focused nursing, patient education, and coordination of care with community resources, while ensuring a flexible, patient-centered approach within CFHW's mission.

Qualifications

• Unrestricted RN licensure in CO or ability to obtain license at time of hire.
• 1 year nursing experience in behavioral health, addiction, or mental health settings preferred.
• Experience with MAT and SUD services is desirable.
• Knowledge of recovery-focused nursing in community health settings.

Responsibilities

• Provide nursing services as part of MAT Outpatient team under ACS.
• Support complex patient care in MAT, withdrawal management, and clinics across CFHW.
• Coordinate care with interdisciplinary team to engage patients in recovery.
• Educate patients and families about MAT, health maintenance, and safety.
