Job summary

Team Select Home Care seeks an experienced Case Manager (RN) to oversee patient care in home health settings under physician orders and care plans. You will report to the Nursing Supervisor and coordinate with families, physicians, and other providers.

The role requires developing care plans, conducting visits, documenting data sets (OASIS, PAR), and ensuring compliance with state payers. Strong communication and clinical leadership are essential.

Qualifications

• Graduate of an approved nursing program and licensed in the state(s) of practice.
• Minimum 1 year nursing experience; home health experience preferred.

Responsibilities

• Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
• Provides case management to an assigned group of patients and staff.
• Submits required data collection sets (OASIS, PAR) within required timeframes.
• Performs initial evaluations and regular reevaluations of patient nursing needs.
• Coordinates care between patients, families, and providers across settings.
• Administers medications per policy and physician orders.
• Maintains confidentiality and completes documentation promptly.
• On-call duty nights, weekends, and holidays as assigned.
