Job summary

Your Health Organization in the Charleston area is seeking a Registered Nurse to serve as a Visiting Nurse (Nurse Case Manager). You will travel to patients’ homes and facilities to evaluate needs and initiate telemedicine visits, guiding care within applicable laws and policies.

This is a full-time, salary-based position with 8-hour shifts, Monday–Friday, 8am–5pm. Responsibilities include coordinating with providers, enrolling patients in Care Management, and developing Plans of Care, while

Qualifications

• Licensed nurse in good standing.
• Minimum of three years of clinical experience.
• Experience in community settings preferred.
• Strong written and verbal communication with care teams.
• Must be able to travel daily with insured vehicle.

Responsibilities

• Facilitate receiving medical records from the patient’s providers.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate and manage a Care Management Plan of Care.
• Coordinate care with providers, facilities, and families.
• Document Care Management activities accurately and timely.
• On-call may be required.
