Bring active RN licensure, strong communication, and case-management experience. This role involves travel up to 90% and requires HIPAA compliance and patient-centered collaboration.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of operation.
• CCM preferred.

Responsibilities

• Provide medical case management to patients through coordination with the patient, physicians, other health care providers, the employer, and referral sources.
• Utilize Case Management steps: assessment, planning, implementation, evaluation, and outcomes.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
