Job summary

TRIUNE Health Group in Philadelphia, PA seeks a Nurse Case Manager to coordinate resources for catastrophically or chronically ill individuals, ensuring timely return to work and holistic care.

The role requires RN licensure in the state, strong leadership, and case-management skills; travel to client sites is expected; a valid driver’s license is required.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of operation.
• Fluency in English (speaking, reading, and writing).
• Three or more years of recent clinical experience, preferably in trauma, psychology, emergency, orthopedics, rehabilitation, occupational health, and neurology.
• CCM preferred.

Responsibilities

• Provide medical case management coordinating with patients, physicians, employers, and referral sources.
• Use case-management steps: assessment, planning, implementation, evaluation, and outcomes.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
• Coordinate delivery of durable medical equipment and home health services; ensure clear communication.
• Assess rehabilitation facilities and arrange transportation and home modifications when needed.
• Maintain HIPAA-compliant communication with patients and families.
• Stay updated on health-care laws (HIPAA, FMLA, ADA, etc.).
• Utilize technology to prepare organized, timely reports.
