Job summary

TRIUNE Health Group is seeking a Nurse Case Manager in Richmond, VA to coordinate care for catastrophically or chronically ill individuals and facilitate timely return to work. The role requires collaboration with patients, families, physicians, employers, and providers to develop cost-effective, holistic treatment plans.

The ideal candidate holds an RN license, with 3+ years of clinical experience; CCM certification is preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of operation.
• CCM preferred.

Responsibilities

• Provide medical case management through coordination with patient, physicians, and employers.
• Apply case management steps: assessment, planning, implementation, evaluation and outcomes.
• Evaluate treatment plans for medical necessity and cost-effectiveness.
• Coordinate durable medical equipment and home health services; ensure clear communication.
• Assess rehabilitation facilities and arrange transportation and home modifications when needed.
• Maintain HIPAA-compliant documentation and clear patient communications.
