Job summary

TRIUNE Health Group in Nebraska seeks a Nurse Case Manager to coordinate resources and design cost-effective, individualized treatment plans for catastrophically or chronically ill or injured individuals, including return-to-work planning when appropriate. You will collaborate with patients, families, physicians, and employers, using HIPAA-compliant processes.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure in the state of operation.
• Three or more years of recent clinical experience.
• CCM certification preferred.

Responsibilities

• Provide medical case management coordinating with the patient, physicians, employers and referral source.
• Utilize the Case Management steps: assessment, planning, implementation, evaluation and outcome.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
• Coordinate delivery of durable medical equipment and home health services; ensure clear communication.
• Travel up to 90% and communicate medical information clearly to patients and families.
