Job summary

TRIUNE Health Group seeks a Nurse Case Manager to coordinate resources for catastrophically or chronically ill individuals, enabling goal-oriented, holistic treatment and timely return-to-work when appropriate.

The role requires active case management across care teams, careful evaluation of plans, and strong communication with patients, families, and providers. Travel up to 90% is expected; RN licensure in TX and CCM preferred.

Qualifications

• Graduate of an accredited school of nursing and RN licensure in the operating state.
• Three+ years of recent clinical experience, preferably in trauma, psychology, emergency, orthopedics, rehabilitation, occupational health, or neurology.
• CCM, CRC, CRRN, LPC, COHN, COHN-S or CDMS certifications are valued.

Responsibilities

• Provide medical case management coordinating with patient, physicians, and employers.
• Utilize Case Management steps: assessment, planning, implementation, evaluation, and outcomes.
• Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
• Coordinate delivery of durable medical equipment and home health services.
• Assess rehabilitation facilities and arrange transportation as needed.
• Communicate medical information clearly to patients and families.
• Stay current with healthcare laws including HIPAA, FMLA, STD/LTD, workers’ comp.

Education

Graduate of an accredited nursing program

Current RN licensure in the state of operation

Fluency in English (speaking, reading, and writing)

Three or more years of recent clinical experience (trauma/psychology/emergency/orthopedics/rehabilitation/occupational health/neurology)

CCM certification preferred
