Job summary

Medical University Hospital Authority (MUHA) seeks a Nurse Case Manager I to coordinate the interdisciplinary plan of care from admission to discharge and to serve as a clinical expert in discharge planning and utilization review.

The role reports to the Manager of Case Management and Care Transitions and emphasizes resource utilization, timely discharge, and collaboration with care teams and payors. PRN, 18 hours/week, hourly pay.

Qualifications

• Bachelor's degree in Nursing from an accredited school of nursing is required.
• Three years of work experience as a Registered Nurse required.
• Prior leadership experience preferred.
• One year of case management and/or utilization review work experience preferred.

Responsibilities

• Coordinate the interdisciplinary plan of care from admission to discharge.
• Monitor care and act as liaison between patient/family, healthcare personnel and insurers.
• Evaluate patient needs and resources; develop and facilitate the discharge plan.
• Identify care issues and suggest revisions to clinical pathways to improve quality of care.
• May conduct utilization reviews.
