Job summary

Medical University Hospital Authority (MUHA) seeks a nurse case manager to facilitate continuity of care for admitted patients. You will coordinate with clinicians to ensure appropriate admission and ongoing review, and develop individual care plans that align with patient needs and insurance resources.

The role requires nursing experience, strong planning and communication skills, and ability to work independently in a high-demand hospital setting.

Qualifications

• Must possess a minimum of two years full-time direct care experience and/or hospital based case management experience.
• Bachelor degree in nursing preferred.
• Must be proficient with Microsoft Word, Excel, PowerPoint and Access; Must have excellent planning and organizational skills; Must have excellent written and verbal communication skills; Must have satisfactory work processing skills; Must have the ability to work independently, with little or no supervision required.
• Required Licenses [South Carolina, United States] Case Manager BLS

Responsibilities

• Reviews past history and conducts case screening within 24 hours of admission.
• Interviews patient and significant others identifying specific health and psychosocial needs.
• Reviews current status and treatment plan; identifies barriers to wellness; determines resources and insurance benefits.
• Develops an individualized care management plan with patient and family input.
• Assesses age-specific and special needs; teaches patient and family as per goals and objectives.
• Identifies options to meet patient needs and determines best course of action.
