Job summary

UCSF Health is seeking a Case Manager to promote optimal clinical and resource outcomes and to facilitate appropriate lengths of stay for hospital admissions. The role requires coordination of complex discharge planning for a designated caseload and active participation on the interdisciplinary team.

The candidate will provide age-appropriate assessments, data interpretation, and interventions, and work with diverse patient populations while supporting quality improvement initiatives across UCSF

Qualifications

• RN case manager with nursing background
• Experience coordinating discharge planning
• Strong written and oral communication skills
• Ability to work in high-volume environment
• Familiar with electronic health records and cross-team collaboration

Responsibilities

• Coordinate and facilitate appropriate lengths of stay for hospital admissions
• Act as a consultant to clinical teams and service lines
• Lead discharge planning for assigned caseload
• Provide age-appropriate assessments and interventions and support quality improvement initiatives
