Job summary

Universal Health Services (UHS) is expanding its case management services at Northwest Reno facility, offering a part-time 24-hour per week role focused on coordinating discharge planning and patient care from admission to discharge.

The Case Manager will assess, plan, intervene, monitor and evaluate care, collaborating with physicians, nurses and social workers to ensure high-quality, efficient transitions and referrals across the hospital network.

Qualifications

• Graduate from an accredited school of nursing; a baccalaureate degree is required and a master's degree is preferred.
• Current Nevada Registered Nurse License.
• Five+ years of nursing experience and at least five years in leadership.

Responsibilities

• Oversee the case management process from admission to discharge within the hospital.
• Collaborate with health-care team to ensure discharge planning and appropriate referrals.
• Provide consultation and education to facility leadership and staff on case management practices.
